Does he want you to be with him? There's not much you can do if he's lukewarm about your relationship and moving out of the state.
This sounds harsh, but sometimes love isn't enough. Love can't fix bipolar or make people communicate freely -- and it certainly can't ignore being left alone for months at a time. Even if it's "normal" for people with bipolar to isolate themselves for weeks, it's not normal for a relationship.
You know you shouldn't be together, you know your kids are being hurt by all of this, and you're exhausted by the whole thing.
My advice is to concentrate on your kids and your own mental and emotional health, and let him go.
